Use the following form to contact the Office of College Advancement regarding alumni, parent, and friend data only. Requests may be made only by Hampden-Sydney Faculty or Staff. Please be as detailed as possible. If a similar report was requested in the past, please note in the "Special Instructions" section. All reports will be delivered by email in Excel format. Efforts will be made to meet the requested deadline but may be affected by periods of high volume.

The information in these reports is proprietary and confidential. Using or disclosing information for any reason other than the legitimate pursuit of employment duties constitutes misuse.
